How to Access our NP Services

For primary care including prescriptions, health screenings, lab work and chronic disease management, we offer yearly memberships. Details for the yearly membership are below. Our goal is to provide comprehensive and thorough primary care at a cost that is accessible and manageable.

How much is the membership fee?

Our membership fee is due when care starts, and is renewed annually.

  • Individual Adult Membership: $1375.00

  • Dependents under 18 years of age: $675

  • Plans can be paid yearly or monthly. The monthly payment option incurs a service charge.

  • Primary care appointments for all ages with any member of our Nurse Practitioner team

  • Comprehensive and extended appointments, including in-person and telephone appointments

  • Labs, diagnostic imaging and specialist referrals

  • Prescriptions and medication management

  • Personalized health navigation including mental health and women’s health

  • Expert medical opinion and second opinions
